Become a Patient

We welcome new patients. We will contact you to schedule an appointment once a referral is received. New patients are most often referred to us by their primary care physician, neurologist, psychiatrist or other healthcare/social service professional. New patients can download and complete our patient registration forms ahead of time in order to speed up the registration at your initial visit. You can also contact us by phone at any time prior to your appointment to address questions or concerns.

Choosing Us For Evaluation Services

We would be honored to be of assistance to you with evaluation and assessment services. Choosing us is simple. If you are experiencing memory loss or recovering from a mild or more severe brain injury, or have questions about possible attention deficit concerns or psychiatric illness, in most instances you should first discuss these issues with your physician. Ask if a more detailed evaluation of your symptoms and concerns might be helpful to clarify your diagnosis and determine the most effective approach to your treatment. We are generally able to schedule new patients within a few weeks or less of receiving a referral from your doctor or other healthcare/social service professional with whom you are involved.

Insurance Plans

We are participants with Medicare and many of the major insurance providers in the area. Please feel free to contact us directly to inquire about our participation with your insurance. It should be noted that in some circumstances, evaluation services are needed but are not medically necessary (e.g., for educational or vocational reasons, or for court-ordered/forensic purposes). Prior to scheduling an appointment, we will assist you in clarifying these issues and making payment arrangements.
